On the contrary, digging one's knuckles into the sole of the foot is a common massage therapy technique, and gives quite a pleasant feeling. The few muscles that cross the sole of the foot are buried deeply beneath the plantar fascia (the web of tissue that surrounds muscles and tendons). It takes much force to work through the fascia to get to the muscles.

I'm a licensed massage therapist, if anyone would like a free demonstration.

from the look of it IT REALLY SEEMS TO HURT

I visited China two years ago, and my friend in Shanghai took me, twice, to a foot-massage parlor.

There are chains of these in China: the one I visited was called "Lotus Foot Massage," I think.

It was one of the most delightful experiences you could wish for, and feels extremely pleasant and relaxing.

It was quite an elaborate service, about 100 minutes long, beginning with a long, hot herbal soak, and then involving vigorous massage not only of the feet, but of the hands, lower arms, parts of the back, and legs. Oh, and a little fellow comes around to trim your toenails, with a sharp knife, which is a tiny bit scary, but then you realize he is better at handling that knife than you are with clippers.

I enjoyed it so very much that I eagerly agreed to go back for another one a few days later. I talked a little bit (my friend interpreting for me) with the little fellow who was massaging me (you may request a masseur or masseuse), about how long a training he had had, how many years he had been doing this work, etc. It seems to be very hard work, to me: by the time we would get toward the end of the treatment, my little masseur was perspiring and panting quite a bit.

It didn't hurt in the slightest, except for the good sort of hurt feeling you have from a good massage, it was not too rough, and it was filled with sensations that were quite new to me. One manoeuver I remember finding intensely pleasant was when my masseur suspended my calf between my seat and his knee, and then used his fists to pound upward into the large calf-muscles, very quickly, and making an unusual "crackle" sound with his knuckles as he worked. This fast-paced drilling is almost too delicious a sensation to bear, if you know what I mean, and I was unable to refrain from some moaning that must have sounded indecent enough to make my stern-faced little masseur crack a smile and laugh a little at how much I was enjoying what he was doing to me.

You leave these sessions feeling extremely relaxed and very, very refreshed, and I'm just sorry that it probably isn't possible to find the same thing in the U.S., and certainly not at a comparably low price.

Those foot-masseurs are very professional, and very good at their work, and there's absolutely nothing indecent going on at these places, which are sprinkled all over the cities, the way we have barber-shops and women have nail-salons and beauty-parlors.

Definitely a not-to-be-missed experience if you happen to visit China.

I can agree with Scooter to a point. I'm also a licensed massage therapist and using a knuckles on the sole of the foot is a common massage technique. I can be pleasant. However, the soles of the foot can hold just as many tender trigger spots as the neck, low back, or anywhere else. Firm, moderate pressure can be relaxing and feel great, but I'm guessing "extreme foot massage" included grinding and twisting a knuckle (or t-bar) deep into the plantar fascia which can be as painful as all get out.
